About The Position

The Community Health Aide supports Gun Lake Tribe Citizens who require assistance with the activities of daily living. The Community Health Aide also serves as a liaison between the Tribal Community and the Health and Human Services Department, keeping agencies/programs, (both Tribal and current outside resources), aware of community needs and what resources are available to Gun Lake Tribal Citizens.

Requirements

  • High School Diploma or General Education Degree (GED)
  • State of Michigan Nurse Aide Certification
  • Previous experience in Tribal Health
  • Working knowledge of Native American culture and traditions
  • Knowledge of primary healthcare, preventative health, medical terminology, childcare, nutrition and/or first aid
  • Ability to work with all age groups
  • The ability and willingness to acquire, or have already received, the necessary medical knowledge, skills, and training to perform the duties essential for this position
  • Must be able to maintain confidentiality according to Tribal, State, Federal and HIPPA guidelines
  • Ability to read, analyze and interpret professional journals, technical procedures and governmental regulations
  • Must possess a strong work ethic including being service orientated
  • Must have excellent interpersonal, self-starter, organizational and time management skills
  • Possess effective oral and written communication skills
  • Must have a high level of accuracy and attention to detail
  • Ability to manage multiple tasks and adjust to changing priorities in a professional manner
  • Excellent computer skills, including proficiency in the use of Microsoft Office programs
  • Possess the ability to operate standard office equipment
  • Ability to work in a team-orientated environment and maintain an effective team working relationship with co-workers
  • Knowledge of electronic health records and health record documentation.
  • Must possess a valid driver’s license, reliable transportation and be insurable
  • Current Basic Life Support (BLS) required or completed within three months of hire

Responsibilities

  • Make home visits to Tribal Citizens with emphasis on the elderly and those with chronic health needs, to carry out care plans which may include assisting Tribal Citizens with the activities of daily living (ADLs) and instrumental activities of daily living (iADLs).
  • Make visits to community members in hospitals or other institutions, as needed
  • Transport clients to health and human services-related appointments/services when deemed appropriate
  • Perform pharmacy pick-ups and medication drop-offs
  • Assist in planning and implementing outreach/health promotion and disease prevention activities and community events
  • Assess community needs and make recommendations for program improvement and/or development
  • Train to implement the Injury Prevention Program with the assistance of current outside resources
  • Provide health education and patient reference materials to clients to assist them in understanding and increasing positive health practices/outcomes
  • Assist Citizens with the applications/certifications or re-certifications for Tribal and outside programs
  • Assist with the organization and implementation of specialty clinics and general clinics when necessary
  • Respond to referrals from Tribal Programs and community members
  • Refer clients to other health or human service resources, as needed
  • Serve at all times as a Goodwill Ambassador from the Tribe
  • Develop and maintain working relationships with outside agencies/programs to access those resources
  • Represent the Tribe’s position if requested, in meetings with other Tribes or with funding agencies
  • Recognize and be sensitive to the cultural diversity inherent in the practice of self-care/health/wellness within Native American communities and educate appropriately
  • Data input and Electronic Health Record maintenance and competent record keeping
