About The Position

The Community Health Advocate I (Bridging the Gap) position at Baywell Health focuses on conducting outreach to identify and address unmet social support needs within the community. The role involves providing resource and referral assistance to patients and advocating for the coordination of healthcare and social support services. The advocate will work under the supervision of the Community Health Program Manager and will engage with community members to promote healthy lifestyles and behaviors.

Requirements

  • High school diploma or equivalent (required).
  • Bachelor's degree in public health, health education, or related field (preferred).
  • 1-2 years in a community health setting or equivalent education/experience combination (required).
  • Strong understanding of health equity, chronic disease prevention, and social determinants.
  • Basic computer skills (Excel, Microsoft Word).
  • Excellent verbal and written communication skills.
  • Bilingual proficiency is highly desirable.

Nice To Haves

  • Experience working with underserved, minority populations (preferred).
  • Familiarity with healthy lifestyle practices, nutrition, and fitness.

Responsibilities

  • Identify patients with unmet social determinants of health needs using screening assessments.
  • Conduct community assessments/screenings to identify disparities in unmet social needs and common health issues.
  • Assist community members with applying to Medi-Cal at health fairs and community events.
  • Coordinate health services and assist with referrals as part of an integrated health care team.
  • Conduct outreach to promote healthy lifestyles and behaviors to targeted populations.
  • Engage with community members to encourage participation in health programs and gather feedback for improvement.
  • Attend meetings with county-wide agencies to coordinate health services for low-income families.
  • Participate in training and care coordination meetings.
