The Community Habilitation Worker assists individuals with disabilities in achieving greater independence and self-fulfillment. Responsibilities include implementing individual's community habilitation plans, assisting and training in daily living skills such as: cooking, cleaning, shopping, and assisting program participants in developing a strong personal network within their community. Comply with all agency standards, rules, practices, policies, procedures, and medication administration. The position is part-time. Work schedule includes evenings and weekends.
