About The Position
The Community Habilitation Specialist will provide assessment, training, and assistance to developmentally disabled individuals enrolled in the HCBS Waiver program. This role focuses on facilitating independence, individualization, productivity, and community inclusion for consumers in their homes or community settings, based on their Community Habilitation Plan.
