About The Position

The community embedded social worker provides clinical services/counseling, screening, and case management services to patients/clients/participants in one or more specific embedded community settings. This includes distinct jobs in which services are provided at primary care or specialty medical practice offices, working with persons who are unhoused, or providing team-based care to a specific population or setting. The role is typically part of an interdisciplinary team that may include medical providers, case managers, peer support services, housing advocates, and others. The work environment is often fast-paced and requires the ability to pivot according to the needs of the team and the clients served.

Requirements

  • Bachelor's degree in social work or other behavioral health discipline required.
  • Master's degree in clinical social work preferred.
  • Substance use counseling experience and expertise required.
  • CDAC, LACD, or comparable certification in the state of Vermont is required.
  • If unlicensed, must be willing to work toward licensure.
  • Knowledge of and experience with the population being served.
  • Possession of a valid driver's license and access to a private vehicle.
  • Working knowledge of computer skills.

Responsibilities

  • Assess patients comprehensively according to the needs of the embedded team and agency.
  • Conduct screening and assessment of patients' social and family structures in context of presenting problems.
  • Plan for patients' aftercare needs.
  • Conceptualize patients and systems in their social context, considering stressors and interpersonal problems.
  • Demonstrate understanding of addiction and recovery, including screening, assessment, and clinical interventions.
  • Synthesize and convey relevant clinical information orally and in writing.
  • Assess and diagnose patients using DSM 5 criteria.
  • Form therapeutic relationships with patients and their families.
  • Provide direct clinical care to promote change according to treatment plans.
  • Engage in individual psychotherapy, family therapy, supportive therapy, and group therapy.
  • Document all interactions with patients in the electronic medical record.
  • Maintain contact with referral sources and community agencies according to aftercare needs.
  • Engage actively with community support systems and coordinate care and treatment.
