About The Position

Swank Center for Memory Care and Geriatric Consultation is looking to hire a Community Educator. The Swank Center has been selected as a GUIDE model by the Centers for Medicare & Medicaid Services (CMS), enhancing its ability to provide comprehensive dementia treatment plans and provide necessary resources to caregivers. As the Community Educator, you will utilize the GUIDE Model, a pilot program by the Centers for Medicare/Medicaid Services, to enhance access to services, provide health education, and assist community members in navigating healthcare and social services systems for dementia patients and their caregivers. Starting July 2025, the Centers for Medicare & Medicaid Services (CMS) will enroll patients in the ChristianaCare Guiding an Improved Dementia Experience (GUIDE) Memory Program, providing ongoing dementia care services and caregiver support. This nationwide initiative aims to enhance the quality of life for dementia patients, help them stay at home longer, and reduce the burden on unpaid caregivers, often family members. The program includes expert clinicians, the latest Alzheimer's treatment, and a multidisciplinary team of board-certified physicians, social workers, nurse practitioners, and educators. Services include comprehensive assessments, personalized care plans, medication management, monitoring and support, care coordination across healthcare services, caregiver education, training, and connection to community and peer support groups.
