Community DSP/Caregiver Part-Time Reports to: Director of Community Integration Location: Cedar Lake Residences (CLR) Jefferson County area Initial orientation and training requirement of 5 full weekdays in La Grange, KY before beginning regular schedule in Louisville. PURPOSE This role is for providing community living supports, personal assistance and homemaker services to individuals participating in Cedar Lake Residences’ (CLR) independent and supported living programs. Facilitation of services is to occur in both in-home, in the community at large and CLR owned properties and support will be based on individual need. This role must function as a member of the CLR team and may provide supplementary assistance to any individual served by CLR. ESSENTIAL FUNCTIONS 1. Implements the Plan of Care (POC) with the objective of increasing self-reliance and independence. Encourages community participation and facilitates cultivation of friendships. 2. Provides guidance to encourage a safe and sanitary living environment independently or with minimal supervision regarding daily living skills, hygiene, housekeeping, cooking/meal planning, and personal assistance related to the individual’s POC. Facilitates involvement and discovery of interests, hobbies and building relationships with others in their community. 3. Assists with services related to coordination and comprehension of medical appointments and pharmaceutical needs. 4. Maintains production standard of 85% billable time, as directed by Director. 5. Facilitates transportation coordination/assistance to accomplish community goals, activities and scheduled appointments. Provides financial education and/or monitoring of personal finances surrounding budgeting responsibilities based on individual need. 6. Encourages and assists individuals in exercising their rights while in community settings. Facilitates constructive ways to respond to conflict and develop strategies for resolution. Provides guidance and direction on social/judgement and health related issues that pertain to integration in the community. 7. Maintains documentation according to specific waiver requirements, which includes a full description of services rendered that entails a summary reflecting progress, outcomes, dates of services, implementation, goal projection and barriers/concerns with services. Helps with preparation of paperwork needed for HUD and Medicaid within specified timeframes. 8. Attends in-services, staff trainings and meetings as scheduled. 9. Represents CLR in a professional manner providing ethical standards of practice for individuals we support. 10. Performs other duties as assigned. QUALIFICATIONS Education : High school diploma Experience : One-year experience working with individuals with ID/DD is strongly preferred Valid Kentucky driver’s license required WORK SCHEDULE Schedules are driven by the needs of the individuals we support. Scheduling is done in conjunction with staff, coordinator and supported individuals. Evening and weekend work as scheduled. EMPLOYMENT STATUS Non-exempt, hourly, part-time, 25 hours per week. Flexible scheduling needed. PHYSICAL DEMANDS While performing the essential duties of this job, the employee is required to: frequently stand, walk, sit, reach outward occasionally reach over shoulder, squat or kneel, bend and climb. (with or without assisting devices or with assistance from others) frequently lift/carry up to 20 pounds, occasionally to 50 pounds. frequently push/pull up to 40 pounds Other physical abilities required are near, close, distant and peripheral vision (corrected) with depth perception and color recognition; a good sense of hearing, smell, touch and the ability to wear personal protective equipment. Normal working conditions will be at an individual’s home, community setting, with occasional work at a day training program or business with moderate noise level.
