About The Position

The Community Diabetes Technician will promote diabetes awareness of the benefits of living an active lifestyle to prevent diabetes and complications and will conduct outreach to provide support to individuals affected by diabetes.

Requirements

  • High school diploma/GED with experience in health, nursing or other related field.
  • A two (2) year degree in health, nursing or other related area is preferred.
  • Must have the ability to communicate with the public.
  • Must have a valid driver's license and meet insurability requirements.

Responsibilities

  • Conduct outreach activities including but not limited to issuing referrals to at-clients to Indian Health Services for further evaluation.
  • Provide notices and following up with clients regarding upcoming events.
  • Assist with coordinating activities with the Diabetes Team, other programs, and schools with diabetes related education sessions and community events.
  • Assist the Diabetes Educator and PHN's to ensure adequate care is being received by referred individuals with diabetes.
  • Maintain records of client referrals, encounter forms, call log, client follow-up and submit timely monthly reports as required.
  • Assist in data entry for community events and school screenings.
  • Perform other tasks associated within the Scope of Work of the Standing Rock Diabetes Project Grant Goals and Objectives.
