Job Overview The Coordinator, Community Dental Home (CDHC) is responsible for increas ing access to dental care for children, perinatal patients, and adults. The CDHC duties include, but are not limited to, oversight (parental/family education), dental home referral coordination, evaluation , training, onsite face-to-face with patient (PSA), transport portable dental units, and mobile driving as needed. The CDHC facilitates the availability of caries prevention and restorative services to families of young children , adults, and p erinatal patients who are affiliated with AltaMed m edical clinics without a dental home. In so doing, the CDHC facilitates referral to one of AltaMed’s d ental c linics and Dental Mobile Units . The CDHC works closely with CPSP, CPHW, m edical and dental p roviders, support staff, a dministrators, d ental supervisors, p arents, c hildren, and c ommunity members. The primary goal of the CDHC is to influence a change in culture for parents, AltaMed teams at the medical clinics, school-based programs, along the communities they serve. This role promotes early intervention for the “oral prevention program”. The CDHC increases parents’ awareness in such matters as education about anticipatory guidance, oral disease, and improves oral health outcomes for children , adults, and p erinatal patients. The CDHC reflects a standardized and coordinated process for Medical/Dental Integration, school-based program, and community-based program, including internal and external referrals. The CDHC will provide clinical preventive , restorative, and major services as needed, assist in follow-up to referrals for dental treatment, and report on activities to achieve LEM goals.

Requirements

  • High school diploma or equivalent with a minimum of a 1-year of experience as a customer service agent, greeter, or receptionist role, or similar experience volunteering (AltaMed Fellow) preferred.
  • Proficiency in MS Office applications, including MS Word, MS Excel, and Outlook, is required.
  • Experience with the Electronic Dental Records system.
  • Driver's License, if assigned to Mobile Units, required.
  • Successful completion of background DOJ/FBI clearance within 30 days of hire, if assigned to Mobile Units, is required.
  • Bilingual English/Spanish also required.

Nice To Haves

  • Experience in Dental and FQHC preferred.
  • Bilingual English/Spanish highly desirable.
  • A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.
