Three (3) years of experience working with people with disabilities or in a nursing home or health care facility

Responsibilities
Develop, provide, and coordinate daily activities for adults, including daily living skills, leisure, recreational, educational, and prevocational activities
Possess knowledge of varying disabilities and their impact on functional and learning capabilities
Ability to work independently and creatively, problem solve as needs arise for individuals and staff that you support
Possesses ability to break tasks down to achievable components
Ability to perform case management duties
Observe and record pertinent information and performance of assigned individuals
Evaluate the performance of adults with disabilities to make recommendations concerning goals and objectives
Monitor individuals assigned and address needs, behaviors, and personal care requirements
