TK Health Clinics delivers essential healthcare to underserved populations in correctional settings across Alabama, Arkansas, Colorado, Kansas, Louisiana, Missouri, Montana, Oklahoma, and Texas. We operate in facilities housing between 25 and nearly 2,000 inmates and offer our staff a secure and unique environment to use their skills to the fullest. Position Overview: We are seeking a Community Coordinator to support mental health discharge planning for incarcerated individuals at Aurora County Jail. In this role, you will play a vital part in ensuring continuity of care after release, building connections between inmates and the community health resources they need.
Requirements
- Bachelor's degree in Psychology, Social Work, or related field
- Current CPR certification (Healthcare Provider level)
- Ability to pass a background check
- Strong communication, documentation, and coordination skills
Nice To Haves
- Licensed Clinical Social Worker (LCSW) or Mental Health Counselor preferred
- Experience with mental health, discharge planning, or case management is a plus
Responsibilities
- Review new admissions and conduct discharge planning evaluations
- Develop and manage procedures for ensuring continuity of care post-release, including prescriptions
- Coordinate follow-up appointments and community services for ongoing treatment
- Educate and support inmates in completing applications and accessing care
- Serve as a liaison between inmates, medical staff, and outside agencies
- Maintain communication with treatment team, clients, and community providers
- Build and sustain partnerships with mental health and social service agencies
- Maintain and update a discharge resource directory
- Accurately document services and submit required reports
- Provide training and support to new team members as needed
- Ensure compliance with facility and regulatory standards
- Perform other duties within scope of practice as assigned
