  • Serve as a community-based member advocate and resource.
  • Engage and assist members in managing their healthcare needs.
  • Collaborate with the Healthcare Services team to provide non-clinical paraprofessional duties in the field.
  • Meet with members face-to-face or via video call.
  • Assist members in scheduling appointments with providers and arranging transportation for healthcare visits.
  • Help members get prescriptions filled and follow up on missed appointments.
  • Assist members in accessing social services and community resources for housing, food, and employment.
  • Conduct outreach to locate and support disconnected members with special needs.
  • Participate in ongoing or project-based activities requiring extensive member outreach.
  • Guide members in maintaining Medicaid eligibility and accessing financial resources.
  • High school diploma or GED required.
  • Minimum 1 year experience working with underserved or special needs populations.
  • Valid driver's license with a good driving record and reliable transportation required.
  • Bilingual in Spanish preferred based on community need.
  • Knowledge of community resources and healthcare systems.
  • Associate degree in a healthcare-related field preferred.
  • Current Community Health Worker (CHW) Certification preferred.
  • Active and unrestricted Medical Assistant Certification preferred.
  • Familiarity with healthcare systems and community-specific culture.
