Responsibilities

• Serve as a community-based member advocate and resource.

• Empower members to navigate and maximize their health plan benefits.
• Assist members in scheduling appointments, arranging transportation, and following up on missed appointments.
• Help members access social services and community-based resources.
• Conduct outreach to locate and support disconnected members with special needs.
• Participate in ongoing or project-based activities requiring extensive member outreach.
• Guide members in maintaining Medicaid eligibility and accessing financial resources.
