About The Position

Molina Healthcare is hiring for a Field-Based Community Connector in or around the Everett or Marysville area. This role will be the non-clinical support for the Health Care Services team. Duties will include (but not be limited) to the following: Be the 'Face of Molina' to our members. Meeting with them in their homes, or in the community to ensure they are getting the care and services needed. This includes introducing them to the Molina Case Management team and following up with members who have been hard to connect with. This role can expect 50% +/- local travel, meeting with members in the field. (Must have a valid driver's license and a reliable vehicle). Reaching out to members (both in person and on the phone) to conduct post discharge screening. Referring members to the appropriate departments depending on the outcome of the screening. Assisting with Health Risk screenings. Assisting with reviewing additional program and special projects as needed and other duties as assigned.

Requirements

  • High School Diploma or GED.
  • Minimum 1 year experience working with underserved or special needs populations.
  • Valid driver's license with a good driving record.
  • Reliable transportation.

Nice To Haves

  • Associate degree in a health care related field (e.g., nutrition, counseling, social work).
  • Bilingual based on community need.
  • Familiarity with healthcare systems.
  • Knowledge of community-specific culture.
  • Experience with or knowledge of health care basics, community resources, social services, and/or health education.
  • Current Community Health Worker (CHW) Certification preferred.
  • Active and unrestricted Medical Assistant Certification.

Responsibilities

  • Be the 'Face of Molina' to members by meeting them in their homes or in the community.
  • Ensure members are receiving the necessary care and services.
  • Introduce members to the Molina Case Management team.
  • Follow up with members who have been hard to connect with.
  • Conduct post discharge screenings with members in person and on the phone.
  • Refer members to appropriate departments based on screening outcomes.
  • Assist with Health Risk screenings.
  • Review additional programs and special projects as needed.
  • Provide non-clinical paraprofessional duties in the field.
  • Empower members to navigate and maximize their health plan benefits.
  • Assist members in accessing social services such as housing, food, and employment.
  • Conduct outreach to locate and support disconnected members with special needs.
  • Participate in ongoing or project-based activities requiring extensive member outreach.
