Assist students in developing employment skills, community access skills, independent living skills, transportation skills, and social skills. Work closely with high school case managers to provide community resources, including agency supports, employment presenters. Participate in transition planning. Essential Duties and Responsibilities: Assist students with community-based access skills. Assist in high school classroom to develop job search skills, interview skills, and connections to community resources. Document student progress and write reports on student work Assist with organizing and attending adult agency tours. Attend various meetings including: Individualized Education Plan (IEP) meetings, Agency meetings and department and/or team meetings, 18-21 Community Connection Program planning meetings.
