About The Position
At AbsoluteCare, we serve the most vulnerable individuals in America, focusing on those at higher risk for disease or with multiple, complex, chronic illnesses. Our mission is to provide comprehensive care that goes beyond traditional medicine, transforming the doctor's office into a care center that offers a wide range of services under one roof. This includes pharmacy, X-rays, blood lab, nutrition services, and urgent care, all aimed at removing barriers to healthcare. The role of the Community Care Manager (CCM) is integral to our integrated community care team (ICCT), where they provide case management for members with complex needs. The CCM conducts comprehensive assessments, creates person-centered care plans, and supports members in achieving their health goals through various interventions and coordination of services.
