What You’ll Do: Provide community-based case management and rehabilitative services to adults with serious mental illness and co-occurring conditions. Deliver most services in clients’ homes and throughout the community to promote independence and recovery. Develop, implement, and maintain individualized service plans in collaboration with consumers, families, and treatment team members. Coordinate care with hospitals, medical providers, community partners, and other stakeholders to ensure continuity of care. Support clients in improving life domains including housing, employment, education, social relationships, health, and legal matters. Practice assertive outreach to reduce hospitalizations, increase housing stability, and improve overall quality of life. Provide mobile crisis response and participate in an after-hours on-call rotation. Transport clients as needed and respond to community-based emergencies. Complete timely documentation and actively participate in supervision and team meetings. Why Join Us?
