About The Position

Position Summary: Responsible for delivering and providing program support for WeCare…Because You Do, a telephone- and email-based care coaching program designed to assist and support older adults living with chronic conditions and their caregivers. Manage a caseload of assigned individuals and, as necessary, assist with efforts to support recruitment efforts. Essential Duties and Responsibilities: The following description is a general representation of the key duties and responsibilities of the position. Complete all initial and ongoing training to develop a high level of proficiency with WeCare…Because You Do. Deliver WeCare…Because You Do including following all requirements of the evidence-based program and delivery protocol via telephone and computer to organizations and individuals that are clients of the program. Maintain a good working knowledge of issues experienced by persons with chronic health conditions and their caregivers, and services and resources available to them in their communities. Complete high quality and timely case documentation using the web-based Care Consultation Information System (CCIS) and the billing systems used by Benjamin Rose. Establish and maintain effective work relationships with all staff and work cooperatively with the BRI Care Consultation team in the Benjamin Rose Institute on Aging department. Assist with marketing WeCare…Because You Do as a program of Benjamin Rose to organizational and individual customers. Participate in continuing education to acquire professional knowledge, maintain licensing (if applicable), and any other requirements to maintain competency. Other Functions and Responsibilities: Able to work independently on most clinical and administrative matters, while seeking consultation and supervision as appropriate. Maintain accurate and complete documentation in client information database(s). Maintain on-going and current knowledge of community services and resources in the geographic regions served by the program and procedures for linking clients and individuals to services and resources. Maintain knowledge to ensure compliance with all ethical standards, laws, and regulations pertaining to confidentiality, personal health information, and adult mistreatment. Various committees or projects as assigned. Other tasks as assigned.

Requirements

  • Bachelor’s degree in social work, nursing, or other aging service-related field.
  • Current and unrestricted Ohio social work licensure, if applicable.
  • Strong verbal, written, oral and electronic communication skills.
  • Excellent problem solving, presentation, customer service and interpersonal skills.
  • Proven ability to work independently and self-motivated to succeed.
  • Excellent organizational skills and ability to handle multiple tasks and prioritize work.
  • Demonstrated proficiency in use of computer software (e.g. Microsoft Outlook and Office).
  • Knowledge and understanding of the needs of persons with chronic health conditions and their family/friend caregivers.
  • Demonstrated proficiency with electronic health record (EHR) software, database management and/or financial software support.
  • Familiarity and adherence to all ethical standards, laws, and regulations pertaining to confidentiality, personal health information, and adult mistreatment.
  • Demonstrated sensitivity to ethnic and cultural diversity and age-specific challenges of persons with chronic health conditions and their family/friend caregivers.
  • Knowledge of community services and resources in the geographic regions served by the program and procedures for linking clients and individuals to services and resources.
  • Able to deliver service over the telephone and computer and document all activities in the computerized record management system.
  • Minimum three (3) years of experience working with persons with chronic health conditions (preferably older adults) and their family/friend caregivers.
