About The Position

We have an opening for a Community Care Hub Specialist in our (hybrid) Tacoma office. As a Community Care Hub Specialist, you will be responsible for developing and delivering training to Community Based Workers, providing shared learning and peer networking opportunities, and providing network partners with support to meet their contracted milestones and expectations. You’ll also assist the operations team by ensuring that referrals and case assignments are entered, processed, and proceed through care coordination workflows as specified. In times of high referral numbers, you may also provide direct client support to community members and clients in the form of short-term resource navigation and resource brokering.

Requirements

  • Associate’s degree in human services or a related field.
  • Washington State Department of Health Community Health Worker (CHW) Certificate or equivalent CHW core competency training.
  • One to two (1-2) years of experience in processing referrals within program requirements using client management system or similar technology.
  • Minimum of three (3) years’ experience working in communities addressing social and health care disparities. Experience providing social or clinical coordination services in Pierce County preferred.
  • Any combination of education, professional, volunteer and lived experience that provides the necessary knowledge, skills, and abilities to perform the duties and responsibilities.

Responsibilities

  • Provide direct support to community-based workers, ensuring they have access to best practice and other necessary resources for delivering effective community care.
  • Coordinate and facilitate opportunities for the community-based workforce to connect, share knowledge, and provide support to their peers.
  • Develop and deliver training sessions for CBWs to enhance their skills and knowledge in community care practice.
  • Monitor and ensure quality of community-based care coordination practice and documentation in client management system.
  • Communicate appropriately and effectively with network partners to resolve quality issues and concerns.
  • Maintain awareness of current affairs, initiatives, and goals of the community-based workforce and relay this information to the Community Care Hub team.
  • Review, update, and compose departmental policies and procedures pertaining to community-based care coordination as needed.
  • Attend staff meetings, trainings, and any additional convenings to meet professional development goals and organizational needs.
  • When needed, provide person-centered, trauma-informed care coordination support to Pierce County residents according to Community Care Hub policies and procedures.
  • Ensure referrals, service delivery, documentation, and billing requirements are met and appropriately processed.
  • Assign referred clients to Community Care Hub partner organization as appropriate.
  • Efficiently manage correspondence with clients, Community Care Hub partners, referral sources, and other service providers.
  • Support the department in quality assurance and continuous quality improvement (QA/CQI) work, ensuring providers are following standardized, evidence-based guidelines.
  • Work in collaboration with all departments at Elevate Health and with diverse sectors, community members, and other stakeholders to continually improve work processes and enhance service provider and client experience.
  • Work to improve Community Care Hub processes, communication, and client care as it relates to health-related social needs (HRSN).
  • Support the operations of the department and Elevate Health as necessary.
