About The Position

We are seeking a Community Care Coordinator for our senior services program. Schedule: Full time 40 hours a week Monday-Friday Compensation: $62,500 annually Job Responsibilities: Assess needs of at-risk individuals who have spent excessive time in Urgent Care and/or the use of other emergency services, including unmet needs, service gaps and barriers. Serve as a “one stop” resource for coordination of care. Assist in accessing necessary services. Work in collaboration with other service providers to maximize use of existing resources. Coordinate care delivery mechanisms with client, caregivers, providers and others. Complete assessments that may include, but are not limited to, outreach progress notes, intake assessments, case progress notes, care plans, data collection, and provide positive outcome summaries. Participate in all relevant meetings, conferences, and sub-committees as assigned. Educate service providers and others about specific issues that may be impacting client's needs. Establish rapport with other care providers and facilitate meetings as needed to identify and resolve unmet needs, service gaps and barriers. Strategize possible systems change solutions to services. Access ancillary service providers and preexisting programs whenever possible; provide services directly when no other resources are available or when direct service from Community Care Coordinator is indicated. Maintain a positive working relationship with both inter- and intra-Agency sources. Maintain professional ethics and adhere to the NASW code of ethics. Retain state certification and/or licensure as applicable and continue to develop professional skills by attending in-services and conferences. Qualifications: Master’s degree required, in Social Work or related field Five (5) years’ experience providing social work services to the respective community Demonstrate a genuine interest and understanding in working with the at-risk populations Possess ability to function as an integral member of a multi-disciplinary administrative team Valid driver’s license, auto insurance, and reliable transportation What’s in it for you? Requirements

  • Master’s degree required, in Social Work or related field
  • Five (5) years’ experience providing social work services to the respective community
  • Demonstrate a genuine interest and understanding in working with the at-risk populations
