What We’re Looking For Trillium Health Resources has a career opening for a Community Care Coordinator to join our team! The Community Care Coordinator is responsible for providing short-term care coordination targeting those with chronic, unresolved or complex physical, behavioral health and social determinant needs. The position has an essential role in helping the member and/or legal guardian to locate community resources (e.g. housing, transportation, nutritional resources), communicating with providers as applicable to promote coordinated and integrated whole person care.

Requirements

  • High School diploma/GED and six (6) years of experience working with BH/MH/SU/IDD population; OR
  • Associate’s degree and four (4) years of experience working with BH/MH/SU/IDD population; OR
  • Bachelor’s degree and two (2) years of experience working with BH/MH/SU/IDD population; OR
  • Equivalent combination of education and experience.
  • Must have a valid driver’s license
  • Must reside within North Carolina
  • Must be able to travel within catchment as required.

Responsibilities

  • Serve as a source to obtain community-based resources.
  • Assist with coordinating services and support for members.
  • Engage identified members into Care Coordination and utilize data and information from multiple sources to address members’ needs.
  • Provide care coordination to assigned members who may have identified needs with behavioral health, physical health, co-occurring, co-morbid or multi-morbid conditions.
  • Provide education about all available services, natural and community support, treatment options, diagnosis, etc.
  • Facilitate referrals for health-related services, community-based resources and social support services.
