What We’re Looking For Trillium Health Resources has a career opening for a Community Care Coordinator to join our team! The Community Care Coordinator is responsible for providing short-term care coordination targeting those with chronic, unresolved or complex physical, behavioral health and social determinant needs. The position has an essential role in helping the member and/or legal guardian to locate community resources (e.g. housing, transportation, nutritional resources), communicating with providers as applicable to promote coordinated and integrated whole person care.

Requirements

High School diploma/GED and six (6) years of experience working with BH/MH/SU/IDD population; OR
Associate’s degree and four (4) years of experience working with BH/MH/SU/IDD population; OR
Bachelor’s degree and two (2) years of experience working with BH/MH/SU/IDD population; OR
Equivalent combination of education and experience.
Must have a valid driver’s license
Must reside within North Carolina
Must be able to travel within catchment as required.

Responsibilities

Serve as a source to obtain community-based resources.
Assist with coordinating services and support for members.
Engage identified members into Care Coordination and utilize data and information from multiple sources to address members’ needs.
Provide care coordination to assigned members who may have identified needs with behavioral health, physical health, co-occurring, co-morbid or multi-morbid conditions.
Provide education about all available services, natural and community support, treatment options, diagnosis, etc.
Facilitate referrals for health-related services, community-based resources and social support services.
