About The Position

This Registered Nurse- Community Care /Case Management Nurse Manager position is located in the Office of Community Care at the VA Manchester location. This position is full -time at 40 hours per week. Included but are not limited to: Leadership & Supervision: First-line supervisory authority responsible for 24/7 operation of assigned units. Provides coverage for other Nurse Managers as needed. Responsible for staff orientation, training, development, and performance evaluation. Acts as a role model, educator, practitioner, and supervisor, providing leadership to the team. Care Coordination: Coordinates Direct and Community care focusing on patient education, self-management, and customer satisfaction. Collaborates with interdisciplinary team members to implement evidence-based programs and processes. Enhances care outcomes by removing barriers and leading process improvement initiatives. Regulatory Compliance: Ensures compliance with accreditation and regulatory standards. Aligns nursing/clinical objectives with organizational strategic outcomes. Maintains knowledge of and compliance with all regulatory and accreditation standards, directives, policies, and SOPs. Monitors and evaluates patient care outcomes, implementing improvements as needed. Resource Management: Manages staffing schedules, budgets, and resources to optimize unit performance. Equips staff with tools for culturally competent care, promoting a culture of accountability for high-quality, cost-effective outcomes. Cultivates alliances across the care continuum, building trust through engagement and emotional intelligence. Professional Development: Engages in professional organizations to enhance leadership and career growth. Adheres to ethical principles in decision-making, fostering accountability at personal, organizational, and staff levels. Contributes to developing care delivery models and environmental design for accessible, affordable, and equitable patient care.

Requirements

  • 1 full and unrestricted license from any US State or territory
  • Current and former Federal employees must submit copies of their most recent SF-50, (Notice of Personnel Action). The SF-50 must identify the position title, series, grade, step, tenure and type of service (Competitive or Excepted). In some cases, more than one SF-50 may be required to show a higher grade previously held
