About The Position

Requirements

  • Master’s degree in Counseling or Social Work required.
  • Minimum of two years of experience working with adults with Serious and Persistent Mental Illness (SPMI) or children with Serious Emotional Disturbance (SED).
  • Must acquire and maintain required training per agency policy and DMH regulations, including documented attendance of professional development/training or education.
  • Current driver’s license, acceptable driving record, and current auto insurance.
  • Successful completion of background checks, including criminal record, driving record, abuse/neglect, and fingerprint checks.
  • Sedentary work: Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently to lift, carry, push, or pull, or otherwise move objects, including the human body.
  • Repetitive movements of hands, fingers, and arms for typing and/or writing during work shifts.

Responsibilities

  • Complete initial and annual assessments within the electronic medical record (EMR) for individuals entering ACS services.
  • Develop person-centered and strengths-based wellness plans.
  • Provide educational information and skill-building to clients and families regarding daily living skills, education, symptom management, and communication to improve functioning and quality of life.
  • Act as a liaison to community and service organizations, assisting clients in gaining access to necessary wellness and rehabilitative services.
  • Facilitate educational groups utilizing a motivational approach as assigned.
  • Complete all required documentation in the EHR and for the Department of Mental Health.
  • Participate as an integral part of the ACS team, providing crisis services and case management as needed.
