LSMNJ is opening a brand new PACE Center in Plainfield, NJ, serving clients in Union County. As we venture into this brand new program, we are seeking a full-time RN to help get us started! This position offers a salary of between of $85-$95k, a full complement of benefits and paid time off!

Requirements

  • RN License New Jersey
  • CPR
  • Current drivers license and proof of auto insurance
  • Broad knowledge base of health care problems of the frail elderly.
  • Knowledge of common safety hazards in the home and improvements to establish a safer environment
  • Skilled in identifying problems and recommending solutions.
  • Knowledge of home care services, nursing principles and practices with particular reference to the elderly.
  • Ability to perform physical assessments and develop plans of care.
  • Knowledge of health instruction techniques and practices.
  • Skilled in preparing and maintaining records and written reports and responding to correspondence.
  • Excellent organization and documentation skills.
  • Previous case management and leadership experience in Home Health Care
  • Familiarity with community health resources, which are available to participants and families
  • Skilled in developing and maintain department quality assurance .
  • Knowledge of current NJ Home Health policies
  • Skilled in establishing and maintaining effective working relationships with participants, medical staff, staff members and family caregivers.
  • Ability to react calmly and effectively in emergency situations.
  • Work well with all members of the care team
  • Knowledge of elder abuse and reporting
  • Familiar with Dementia, Wandering Behavior and Intervention
  • Knowledge of Standard Precautions & Fire and Emergency Procedures in healthcare setting
  • Knowledge of Participant Rights and Workplace Safety
  • Employees must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
  • Minimum of 1 year of nursing experience in acute care setting or 1 year experience in home care or long term care setting
  • Minimum of 1 year working with the frail or elderly.

Responsibilities

  • Provide comprehensive nursing care to our participants at the home and/or in the center.
  • Perform the initial assessment, annual reassessment and change of condition assessments of participants in the clinic or their home environment and coordinate individualized plans of care with the appropriate resources.
  • Responsible for implementation of physician orders and plans of care
  • Delivering education to participants, caregivers and family
  • Appropriately communicating participant condition with LIFE staff.
