The Community-Based Pharmacy Residency is a one year (52 week) postgraduate program designed to develop advanced skills in ambulatory care and community pharmacy practice. The resident will provide direct patient care to our health center patients, while gaining experience in medication therapy management (MTM), population health, chronic disease management, and leadership development. Learning Opportunities · Focused rotations in diabetes management, asthma care, complex care management, specialty pharmacy, and population health. · Exposure to 340B program operations and compliance strategies. · Leadership training and opportunities to participate in organizational initiatives.
