The Community-Based Mental Health Therapist provides person-centered, recovery-oriented therapy services to clients with moderate to higher levels of need. Services are delivered in the home and other community locations, bringing care to our clients in their natural environments, but may also be provided in office or via telehealth, as appropriate. As part of an interdisciplinary care team, the therapist collaborates with behavioral health colleagues as well as medical, dental, and sexual assault/domestic violence (SA/DV) providers to address the whole-person needs of clients. This role includes supporting continuity of care across service levels, helping clients maintain progress when transitioning to or from more intensive services such as Community Support Team (CST) or other higher levels of care. Therapy services are guided by evidence-based models and are responsive to the cultural, social, and trauma-related needs of the community.
