About The Position

The Community Access Coordinator is responsible for elements of practice leadership that build and support the team to provide high-quality supports that deliver positive outcomes for clients with disabilities across various settings, including site-based services, community groups, individual supports, and in-home assistance within the Western Metropolitan Region . Under the direction of the Community Access Manager, the Coordinator ensures the delivery of innovative and flexible services that meet client needs and organizational goals. This position reports directly to the Manager, Community Access. As a Coordinator within our Community Access Team, you will play a pivotal role in enhancing the quality of life for individuals with disabilities and, where applicable, their families.

Requirements

  • Demonstrated management experience, preferably within a community services environment.
  • Understanding of legislation and standards relevant to the disability sector.
  • Knowledge and understanding of how to support staff in implementing contemporary and person-centred approaches to uphold the rights and support needs of individuals with disabilities.
  • Excellent interpersonal and communication skills, both written and verbal.
  • Demonstrated ability to manage financial and staff resources within a complex environment.
  • Excellent initiative, self-directed work practices, and highly developed organizational skills.
  • A relevant qualification and/or significant experience in the human services area

Responsibilities

  • Assisting in managing services across various settings.
  • Help oversee change management and support service performance.
  • Lead a dedicated team of disability support workers, providing high-intensity support to clients.
  • Ensure the overall performance of the service aligns with annual work plans and meets established targets.
  • Promote person-directed support and community participation that exceeds minimum disability standards, all within available resources.
  • Manage the day-to-day operations of supported accommodation services, surpassing identified Key Performance Indicators (KPIs).
  • Provide high-quality support to customers and their families, offering management support to care staff.
