Reporting directly to the Hospital at Home Director of Nursing, The Command Center Staff RN is responsible for providing high-quality patient care to various patient populations within the framework of the Hospital at Home model. Job Description: Essential Duties & Responsibilities including but not limited to: • Utilizing monitoring, assessment, planning, intervention and critical thinking skills to care for acute care patient populations in the home • Navigating patients and families across the continuum of care • Monitoring and responding to biometric data per Hospital at Home Command Center protocols • Conducting audio/visual patient care visits as directed by the care team and/or protocol • Ensuring all interventions in the patient’s care plan are implemented efficiently with a patient centered approach • Leveraging the resources and expertise of supplier partners to implement collaborative workflows, standards, policies, protocols, guidelines and documentation systems to support safe, reliable, high-quality, evidence-based care with clinical protocols as the foundation • Adhering to the clinical protocols and be willing to obtain the necessary training to provide care within the context of the providing evidence-based care • Promoting professional practice and a culture of safety; willingness to engage in process improvement efforts Clinical Roles and Responsibilities: • Providing protocol driven assessment, care coordination and interventions as appropriate informed by Hospital at Home protocols for diverse patient populations with a focus on oncology, medicine and post- operative care • Exhibiting sound clinical decision making and critical thinking skills • Maintain and practice in accordance with nursing guidelines per state and hospital regulations • Facilitating admissions to l Hospital at Home utilizing Hospital at Home guidelines • Serving as a role model for professional nursing practice • Utilizing nursing knowledge to identify, prevent and/or solve complex acute and/or recurring patient care problems • Sharing expertise with the multidisciplinary team and serve as a resource to staff in area(s) of expertise • Supporting research efforts to advance knowledge and promote evidence-based practice • Preforming other duties and responsibilities as assigned by the Senior Director of Nursing Quality and Safety Responsibilities: • Ensure compliance with reporting requirements for Sentinel Events and Serious Incidents • Participate in quality and safety investigations and root cause analyses • Take appropriate action to ensure patient safety as a result of any investigation; and • Serve as a resource in quality improvement processes and technique Organizational Requirements: 1. Maintains strict adherence to the Lahey Hospital and Medical Center Confidentiality Policy. 2. Incorporates Lahey Hospital and Medical Center Guiding Principles, Mission Statement and Goals into daily activities. 3. Complies with all Lahey Hospital and Medical Center Policies. Complies with behavioral expectations of the department and Lahey. 4. Maintains courteous and effective interactions with colleagues and patients. 5. Demonstrates an understanding of the job description, performance expectations, and competency assessment. 6. Demonstrates a commitment toward meeting and exceeding the needs of our customers and consistently adheres to Customer Service standards. 7. Participates in departmental and/or interdepartmental quality improvement activities. 8. Participates in and successfully completes Mandatory Education. 9. Performs all other duties as needed or directed to meet the needs of the department.

Requirements

• Nursing Diploma or ASN, BSN preferred
• A current Massachusetts RN license
• BLS certification
• Clinical knowledge
• Able to promote patient safety and a positive patient experience
• Flexible and comfortable with a changing and dynamic work environment and schedule across all shifts and days
• Team player who also works well independently
• Participates in ongoing learning and continuing education to care for a variety of patient populations
• Collaborate, advocate and mentor others with a professional, non-condescending manner

• Three (3) years RN experience
