Community Medical Services (CMS) is hiring a Community Impact Specialist to assist patients recovering from addictions to opioids and other illicit substances. Every day you will use your experience in substance abuse rehabilitation and opioid treatment programs to foster relationships with community partners, including community coalitions, health homes, sister agencies, specialty providers, local hospitals, clinics, law enforcement, and fire and EMT services. An integral point of contact for the coordination of care between opioid treatment program clients and community agencies, your work as a community relations specialist will support CMS clinic staff in providing a safe place for anyone looking to start their recovery journey with us. Can interact with clients in a friendly, helpful, and courteous manner. Is comfortable working in a wide range of settings, including any CMS clinic location, community-based settings, correctional health facility, or inside a transport vehicle. Has knowledge of community resources, including food stability, housing, recovery stability, employment, benefits, inpatient MAT friendly recovery services, shelters, emergency services, PCP, and special populations. Works with passion, humility, integrity, and a positive attitude. To thrive in this role, you must be able to: Identify and access community resources. Provide timely and accurate documentation, including for all client/agency encounters, coordination of care efforts, and community outreach activities, presentations, and meetings. Maintain outreach program data in multiple databases, including QuickBase, Methasoft, and CRM. Maintain appropriate coordination of care with all systems and provide documentation as requested from system partners. Provide support, including intake documentation, to special populations and projects. Resolve client altercations, manage crisis episodes, and assist in conflict resolution. Attend provider and community meetings for the purpose of advocating, educating, and supporting system partner relationships. Work with the project management team to support program development and implementation. Receive and process referrals for community support from multiple CMS clinics and community system partners. Complete documentation within 24 hours of shift.
