The Cognitive Rehabilitation Therapist II is responsible for evaluation and treatment of Home Community Re-entry (HCR) patients in the clinic and community setting, delivery of cognitive therapy sessions, clinical case management, coordination of the rehabilitation plan, and discharge planning. Primary responsibilities also include: mentorship, supervision of students, and advanced knowledge of the acquisition, application, adaptation, generalization, and maintenance of cognitive skills. The Cognitive Rehabilitation Therapist is an integral healthcare provider accountable for assisting the patient with achieving their highest level of physical and cognitive function and works in conjunction with other health care professions to coordinate care. The Cognitive Rehabilitation Therapist will uphold the mission, vision, and values of Jefferson Health. Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson. Provides direction to clinical treatment team for comprehensive client care Provide evaluations, treatment planning, treatment interventions and discharge planning for a full patient caseload in the HCR outpatient setting. Provides all ancillary/administrative requirements for a full patient caseload, including documentation, billing, scheduling, etc. Demonstrates appropriate communication, professionalism and complies with all hospital and network policies Develops and implements individual and group activities that meet the needs and interests of the HCR patients, with focus on promoting social interaction, activity pattern development, cultural awareness and provides ongoing brain injury education from a cognitive/behavioral perspective to individuals and families living with brain injury Participates in education and teaching commensurate with clinical level. Plans, organizes, and delivers treatment programs that help patients recover and rehabilitate. Utilizes tools and techniques to administer safe therapeutic interventions and trainings, for patients and families. Records and documents patients' current status and progress information for: evaluation, all therapeutic interventions including daily therapy sessions, progress notes, and discharge summaries. Completes clinical ladder requirements commensurate with Level 3. Completes specialized program specific assessments, evaluations, and feedback. Participates in student education and supervision for part-time and/or full-time students. Provides mentorship and assists with new staff orientation and competency. Shares clinical expertise through educational forums internally and externally. Supports supervisor/manager with ongoing clinical and programmatic initiatives including leadership responsibilities.
