The Coding and Documentation Educator assists with Clinical Documentation Integrity (CDI) as it relates to Risk Adjustment with documentation best practices, understands clinical suspects and appropriate clinical documentation and accurate coding. This role reports to CDI leader and will work closely with providers and operations to effectively provide Hierarchical Condition Category (HCC) education including but not limited to clinical documentation integrity, coding, HCC Risk Adjustment Methodology, and provider performance metrics. The successful candidate will have an advanced understanding of the HCC risk adjustment methodology gained from HCC documentation, coding, and provider education experience.

  • Analyzes medical record documentation for HCC accuracy, correct documentation, and educational opportunities.
  • Provides education to physicians, advanced practice providers, and other key healthcare providers regarding the need for accurate, specific, and complete clinical documentation in the patient's medical record.
  • Serves as a subject matter expert in clinical documentation and coding best practices for both internal and external partners.
  • Participates actively in prospective program development, execution, and performance.
  • Assists the Clinical Documentation Integrity team by making recommendations for process improvements to further enhance program coding goals and outcomes.
  • Evaluates medical records to ensure Monitor, Evaluate, Assess, and Treat (M.E.A.T) criteria support the existence of submitted diagnosis codes.
  • Technical/Vocational School
  • 5 years of coding experience, or experience with clinical documentation reviews and provider education.
  • Registered Health Information Technician (RHIT) - Cert, OR
  • Certified Coding Specialist (CCS), OR
  • Certified Documentation Improvement Practitioner (CDIP) OR
  • Certified Clinical Documentation Specialist-Outpatient (CCDS-O), OR
  • Certified Clinical Documentation Specialist (CCDS), OR
  • Certified Documentation Improvement Practitioner (CDIP)
  • Bachelor's degree
  • Certified Risk Adjustment Coder (CRC)
