The position involves coding primarily for acute hospital inpatient, skilled nursing facilities (SNF), and rehabilitation for the purpose of accurate reimbursement, research, and compliance with federal regulations. The coder will also handle coding for ambulatory, emergency room/urgent care, interventional radiology, and same-day surgery records as needed. The role requires reviewing the entire medical record to code diagnoses and procedures using the International Classification of Diseases, 10th Revision, Clinical Modification (ICD-10-CM) and Current Procedure Terminology (CPT) classifications. The coder will query physicians for clarification when there is uncertainty in the documentation and will analyze and review records for completeness. Active participation in the Clinical Documentation Improvement Team (CDI Team) is also expected.
