About The Position

The Certified Nursing Assistant (CNA) position reports to the Director of Nursing (DON) or RN on duty. The role involves adhering to instructions issued by the DON and performing duties in accordance with the Agency’s methods and techniques, complying with recognized policies, procedures, standards, and practices, and within the State Regulatory Acts. The CNA will follow Standard Precautions and OSHA Guidelines, providing support, supervision, and assistance to clients in their daily living skills.

Responsibilities

  • Bathe clients by multiple methods: tub, shower, bed bath and perineal care.
  • Dress and groom clients including brushing hair, shaving, oral hygiene, and nail care.
  • Measure and report vital signs: temperature, pulse, respiration, and blood pressure.
  • Weigh clients monthly as ordered and record.
  • Provide care that minimizes the risk of skin breakdown by being attentive to hygiene, proper positioning, and injury prevention.
  • Observe and report the presence of skin breakdown to the DON.
  • Properly and safely lift, move, transfer, and position clients in bed, wheelchairs, and while using other equipment.
  • Provide range of motion (ROM) exercises.
  • Assist the family to plan and prepare nutritious meals and snacks.
  • Safely feed clients as ordered, following prescribed feeding protocols.
  • Record client's intake and report any changes.
  • Provide care as detailed in the interdisciplinary care plan.
  • Apply and assist clients with adaptive devices as ordered.
  • Document information clearly and completely utilizing correct forms and in a timely manner.
  • Maintain a healthy, safe, secure, comfortable environment for every client.
  • Plan and organize work assignments; prioritize the specific needs of clients.
  • Accurately observe, recognize changes and communicate or report any changes from the client’s normal status to the DON in a timely manner.
  • Report any incidents, accidents, or any unusual observations, with or without injury, promptly to DON.
  • Monitor clients to prevent wandering from residential sites or grounds.
  • Change bed linens, do laundry, make occupied and unoccupied beds, and perform light housekeeping chores.
  • Assist with certain treatments as ordered by physician and approved and supervised by the delegating nurse.
  • Ensure proper reporting of any change in medical status including behavioral incidents.
  • Administer medications as prescribed, providing appropriate medication assistance as needed.
  • Escort clients to doctor visits and community activities as directed by DON.
  • Assist clients with transferring and ambulating, if needed.
  • Comply with policies, procedures, and specific protocols for client lifts and transfers as designed by the care plan.
  • Utilize safety precautions as ordered for individual clients.
  • Encourage clients to become as independent as possible according to their plan of care.
  • Promote client’s mental alertness through involvement in activities of interest.
  • Provide simple emotional and psychological support to clients and other household members.
  • Demonstrate understanding of boundaries when caring for clients and their families.
  • Interact with clients, families, and visitors in a warm, caring, and courteous manner.
  • Report any changes in the client’s mental and/or physical condition immediately to employer and delegating nurse, if applicable.
  • Carry out assignments as instructed and report to the employer if unable to do so.
  • Interact with other employees and/or home health professionals, such as physical therapists.
  • Follow standard precautions whenever giving any aspect of client care.
  • Understand scope of practice of CNA role and perform within parameters of job description.
