About The Position

The primary purpose of this position is to provide routine daily nursing care and services to assigned residents in accordance with their assessment and care plans, while adhering to facility protocols and under the direction of supervisors. The role involves direct interaction with residents, ensuring their comfort and well-being, and maintaining accurate records of their care.

Requirements

  • Must possess a minimum of a 10th grade education.
  • Licensed Certified Nursing Assistant in accordance with state laws.
  • Ability to read, analyze, and interpret general business and technical documents.
  • Ability to write reports and business correspondence effectively.
  • Mathematical skills to apply concepts such as fractions and percentages to practical situations.
  • Ability to solve practical problems and interpret various instructions.

Nice To Haves

  • CPR certification preferred.

Responsibilities

  • Identify residents before administering treatments and serving meals using wristbands or photo cards.
  • Record information using only authorized abbreviations established by the facility.
  • Report changes in residents' conditions to the Nurse Supervisor/Charge Nurse promptly.
  • Maintain accurate and informative records on flow sheets, notes, and charts.
  • Report all accidents and incidents observed during the shift.
  • Protect residents' health information and report any violations.
  • Perform only nursing care procedures for which training has been received.
  • Prepare residents' rooms for their arrival, ensuring all necessary items are available.
  • Greet residents and escort them to their rooms, introducing them to roommates and staff as appropriate.
  • Make residents comfortable and assist with personal possessions as needed.
  • Transport residents to new rooms or receiving areas as required.
  • Create a warm and calm atmosphere throughout the unit and shift.
  • Meet regularly with nursing personnel to address service improvement.
  • Report complaints and grievances made by residents.
  • Participate in nursing reports as instructed.
  • Follow established policies regarding exposure to blood/body fluids.
  • Assist with daily personal care functions for residents.
  • Monitor and record residents' vital signs and report any changes to the Nurse Supervisor/Charge Nurse.
  • Ensure residents' personal care needs are met and assist with food arrangements as necessary.
  • Perform after meal care and maintain cleanliness of residents' water pitchers.
  • Cooperate with inter-departmental personnel to maintain adequate nursing services.
  • Notify the facility of any lateness or absences from work.
  • Report occupational exposures to hazardous materials as per facility policies.
  • Assist residents with preparing for medical tests and examinations.
  • Observe and report any signs of pressure areas or skin breakdowns to prevent bedsores.
  • Honor residents' requests for treatment refusals and report them to supervisors.
