About The Position

The Assisted Living (AL) Certified Nursing Assistant (CNA) at Sanford Health serves as a primary caregiver to residents in an assisted living setting during scheduled work periods. Under the supervision of a charge nurse, the NA provides resident-centered nursing care and daily living assistance. This includes assisting with basic health care needs, daily living activities such as bathing, toileting, grooming, dressing, obtaining vital signs, and providing psychosocial support. The role also involves assisting with food preparation and dining, ensuring a positive experience for residents, and providing meals, snacks, water, and supplements. Depending on the location, the NA may assist with resident transfers, repositioning, walking, and range of motion exercises. Additional duties can include aiding nursing staff with procedures, collecting non-invasive body fluid specimens, gathering vital signs, and administering prescribed medications as delegated and within state regulations. The NA is responsible for documenting resident care outcomes, medication administration, behavior, activity, and dietary information, and communicating any changing conditions or concerns to the charge nurse. Housekeeping and laundry services for residents' living areas, as well as assistance with wellness and recreational programs, may also be part of the role. The NA is expected to provide age-appropriate care, potentially including care for tenants with dementia, and must adhere to infection prevention and control policies and proper Personal Protective Equipment use. The position operates within a 24-hour nursing department, requiring flexibility in shifts, including weekends and holidays.

Requirements

  • Minimum age of 16.
  • Certified Nursing Assistant (CNA) certification with the State Board of Nursing.
  • Satisfactorily completes yearly department specific competency testing.
  • Maintains a record of continuing education credits for each year (minimum of 12 hours annually).
  • Maintains all department specific required certifications.
  • When applicable, for applicable states, certified and registered with the Unlicensed Assistive Person (UAP) Registry.
  • May require a motor vehicle report and proof of valid driver’s license, and must be medically qualified to perform the essential driving functions of this position as per Sanford Driving Policy (for Sanford Category II Driver roles).

Nice To Haves

  • High school graduate or general equivalency diploma (GED) preferred.
  • Prior clinical or nursing assistant experience in assisted living preferred.

Responsibilities

  • Provides assistance with basic health care needs including daily living activities that may include, but are not limited to, bathing, toileting, grooming, dressing/undressing, obtaining and recording vital signs, and providing psychosocial support and other personal care to assigned resident.
  • Assists the resident in transferring, repositioning, and walking using correct and appropriate transfer techniques and equipment and also provides range of motion and passive exercises (depending upon location).
  • Completes tasks related to food preparation and dining, while creating a positive dining experience for the resident.
  • Assists residents with meals and snacks, provides water and supplements as appropriate.
  • Aids providers and nursing staff members with procedures and/or collect non-invasive body fluid specimens and gather vital signs (depending upon location).
  • Administers prescribed medications as delegated by a licensed nurse and within their scope of practice as defined by state regulations (depending upon location).
  • Documents the outcomes related to resident care, medication administration, behavior, activity, and dietary.
  • Communicates changing condition, medication follow-up and care related concerns/responses to the charge nurse.
  • Provides housekeeping and laundry services to assist residents with keeping living areas clean and operational (depending upon location).
  • Provides assistance and documentation with wellness and recreational programs and encourages residents to participate.
  • Has knowledge of and delivers age-appropriate care related to the physical and psychological needs of the resident as per care/service plan.
  • Is knowledgeable of providing care to tenants with dementia (depending upon the location).
  • Knows and follows infection prevention and control policies and Personal Protective Equipment use.
  • May be asked to provide direction to students when they are providing resident care, treatment, and/or services as part of their training.
