Sonoma Post Acute is growing our clinical team and looking for compassionate, reliable Certified Nursing Assistants (CNAs) who are passionate about quality care and teamwork. Requirements

  • Current California CNA certification
  • Dependable, caring, and team-oriented

Responsibilities

  • Assisting with bathing, dressing, grooming, toileting, and mobility
  • Measuring and recording vital signs and reporting changes in condition
  • Assisting residents with meals, hydration, and post-meal care
  • Maintaining clean, safe, and organized resident rooms
  • Escorting residents during admissions, transfers, and discharges
  • Responding promptly to call lights and resident needs
  • Documenting care in charts, notes, and flow sheets
  • Following all privacy, infection control, and safety procedures
  • Reporting incidents, concerns, or equipment issues promptly
  • Participating in shift reports and team communication
  • Supporting restorative and rehabilitative care as directed
