Requirements

  • High School diploma or equivalent preferred
  • Must have an active Colorado State Certified Nursing Assistant License in good standing
  • Must have BLS and Blood Glucose Monitor training within 30 days of hire
  • 1 plus years of experience preferred
  • Immunizations required for employment; including flu vaccine

Responsibilities

  • Assisting patients during transportation to and from appointments or during initial Swing bed admissions.
  • Ensuring patient safety and comfort throughout the transportation process.
  • Conveying necessary patient paperwork supplied by the nursing staff to the receiving clinic.
  • Returning patient paperwork to the Ward Clerk or floor nurse upon completion of transport.
  • Communicating any additional orders from the physician to the appropriate nursing staff.
  • Supporting the inpatient department with various duties to maintain productivity when not engaged in transportation activities.
  • Reliable and punctual attendance; expected to be at job as scheduled each day.
  • Work harmoniously with Transportation staff and Inpatient Nursing staff to meet all patient needs.
  • Check the Transportation Schedule daily to confirm appointments.
  • Assist with transfer admissions from other facilities (e.g., St Mary's Hospital, Community Hospital, and Foresight).
  • Arrive 15 minutes prior to scheduled pick-up time to assist with the grooming needs of patients.
  • Assist patients with mobility, hygiene, and activities of daily living, including transportation within the facility and to/from appointments.
  • Collect the patient's packet and escort the patient to the waiting transportation vehicle.
  • Safely assist patients with boarding the bus or passenger vehicle, ensuring their comfort and safety throughout the process. This may include operating the buses mechanical lift system.
  • Work collaboratively with the driver to properly secure patients and wheelchairs using vehicle restraints and securement systems.
  • Remain with the patient throughout the transport, providing reassurance, addressing any needs, and ensuring their safety and comfort.
  • Upon arrival at the appointment, ask the patient if they wish the CNA to come into the exam room or wait outside.
  • Provide comfort measures as needed (proper clothing/cover for temperature, position, hydration, toileting, and snacks).
  • Ensure the patient has prosthetic devices needed (eyeglasses, hearing aids, etc.).
  • Communicate necessary information to others as appropriate.
  • Assist with patient admissions and discharges.
  • Support nurses and physicians with medical procedures as directed.
  • Provide basic life support in emergency situations.
  • Accurately obtain, document, and report vital signs, intake/output, and weights.
  • Monitor general condition of patients and report observations and significant changes.
  • Recognize and address safety issues.
  • Observe universal precautions being followed.
  • Maintain a clean and organized environment in patient and staff areas.
  • Communicate courteously and effectively with patients, families, and staff.
  • Protect patient's rights to privacy and dignity.
  • Display courteous and professional interactions with the public, patients, families, and staff.
  • Participate in continuing educational activities.
  • Document accurately and completely within the scope of a CNA job description following facility protocols.
  • Seek assistance from Charge Nurse or DNS in any situation beyond own ability or scope of practice, or if there are questions or doubts.
  • Correctly operate all equipment per facility policies and procedures (e.g., telemetry, LifePak12, EKG, Bladder Scan, Glucometer).
  • Complete proper specimen collection and deliver to Lab for processing.
  • Follow stocking of supplies and reorder procedures, and adhere to patient transfer procedures for Hospital and ER.
  • Promote patient independence and control of their life within their abilities, and report any condition changes, accidents, or injuries immediately to the Charge nurse.
  • Participate as backup active driver as needed.
  • Perform other job-related duties as assigned.
