SUMMARY: Under general supervision, provides direct care to residents and assists with activities of daily living. Provides effective, timely, and quality health care assistance and related support for nursing facility residents. ESSENTIAL JOB FUNCITONS: Direct Care: Assists residents with bathing functions according to resident care plan and as directed (i.e. Bedbath, tub, or showerbath, etc). Assists residents with daily dental and mouth care (i.e. Brushing teeth/dentures, oral hygiene, special mouthcare, etc). Assists residents with undressing/dressing as necessary. Assists residents with hair care functions (i.e. Combing, brushing, shampooing, etc). Assists residents with nail care (i.e. Clipping, trimming, and cleaning the finger/toenails). Assists residents with shaving (female residents- facial hair, under arms, legs, etc. as instructed. Male residents- facial hair) Keep residents dry (i.e. Change gown clothing, linen, etc. when it becomes wet or soiled). Position residents in correct and comfortable positions for pressure relief. Assists residents with bowel and bladder functions (i.e. Take to bathroom, offer portable commode, change Prevails, etc.) Assists residents in preparing for social programs, activities, and appointments (i.e. Church, parties, visitors, doctor’s appointments, etc). Assists residents with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, and lifts, etc. Assists residents to walk with or without self- help devices as instructed. Provides eye and ear care for residents (i.e. Cleaning eyeglasses, hearing aides, etc). Answers resident call lights promptly and ensure that residents who are unable to call for help are checked on regularly. Assist with the care of the dying resident, maintaining hope, dignity, and love for the resident at all times. Provide post-mortem care as instructed. Housekeeping and general cleaning: Changes soiled linen and bedding as required. Disposes of soiled or contaminated items in accordance with facility policy on blood borne pathogens. Make beds as needed. Put extra blankets on the beds as requested. Ensure residents have fresh drinking water during each care delivery. Keeps bed linens tight to avoid wrinkles from forming under the resident. Performs general cleaning of equipment used. Communication/Reporting: Takes vital signs on residents and properly records each resident’s vital signs as assigned (i.e. Blood pressure, temperature, pulse). Observes residents well being including changes in behavior, change in vital signs, etc. and reports such changes to the appropriate supervisor. Maintains intake and output records as instructed. Weighs and measures residents as instructed. Participates in and receives the nursing report on assigned shift. Maintains the confidentiality of all resident care information according to the St. John’s Policy on Confidentiality and Resident Rights. Reports suspected abuse of a resident or inappropriate behavior toward a resident to the charge nurse immediately. Dining/Eating: Assists residents in preparation for meals (take to the bathroom, wash hands, comb hair, place bibs, take to/from the dining room, etc.) Serves food trays and assists with feeding as needed (cutting foods, feeding, etc). Records or charts the resident’s food intake. Serve between meal and bed time snacks as desired by the resident. Perform after meal care (remove trays, wash face and hands, take to the bathroom, brush teeth, clean dentures, etc.). Safety: Obeys safety rules of St. John’s United Identifies, reports, and/or corrects obvious safety hazards. Attends mandatory safety in-services. Utilizes proper body mechanics when transferring residents. Follows established policies and procedures regarding exposure to blood/body fluids. Wear and/or use safety equipment and supplies (e.g., gait belts, mechanical lifts, personal protective equipment). Performs other duties as assigned by Nurse Managers or person in charge of area where work is being done.
