About The Position

The CNA Supervisor provides a balanced combination of direct patient care, staff supervision, and team-level operational leadership to ensure safe, high-quality clinical services and effective team performance. This role maintains a strong clinical presence while serving as a visible supervisor and operational leader who supports staff development, workflow effectiveness, and alignment with organizational standards.

This position is responsible for supporting patients and families across all Unity service areas, ensuring consistent care and presence throughout our entire community network.

Schedule: 5 days per week (variable) – PM/NOC shifts possible with this role, every 3rd weekend, on call rotation 10-15 times per year, 1-2 holidays per year.

Requirements

• High school diploma or GED required.
• Completion of a Certified Nursing Assistant program.
• Must be listed on the Wisconsin Nursing Assistant Registry and be eligible to work at state and federally certified facilities.
• Minimum 3 years CNA experience.
• Prior experience in home health, hospice, or facility/community-based care.
• Prior experience leading others, fostering a positive team environment, problem-solving and decision-making skills, and an effective communicator.
• Valid WI Drivers’ License, reliable transportation, and proof of current automobile insurance coverage.
• Computer proficiency (MS Office – Word, Excel, Electronic Medical Record, Scheduling System, and Outlook)
• Strong communication, organization, and documentation skills.
• Highly motivated and organized with a strong attention to detail and analytical skills to help solve problems.
• Ability to communicate effectively, both verbally and in writing.
• Ability to maintain confidentiality and discretion in business relationships and exercise sound business judgment.
• Ability to keep up to date with policies, evidence-based best practices, and regulations.
• Ability to prioritize and handle multiple tasks and capable of dealing with deadline pressures.
• Comply with all applicable local, state, and federal laws and regulations as well as policies and procedures of Unity.
• Satisfy mandatory training requirements regarding local, state, and federal regulations and/or Unity’s policies and procedures.
• Perform other job-related duties as assigned.
• Ability to adjust to frequently changing demands.

Responsibilities

• Provide direct patient care in accordance with CNA job description (refer to separate CNA job description), CNA certification, assigned competencies, and organizational policies.
• Assist patients with activities of daily living (ADLs), mobility, comfort measures, and personal care needs.
• Observe, document, and report changes in patient condition to licensed nursing staff in a timely manner.
• Maintain current skills and competencies relevant to assigned patient populations and programs.
• Collaborate with nurses and interdisciplinary team members to support coordinated, patient-centered care.
• Ensure documentation is accurate, timely, and compliant with organizational standards.
• Support patient coverage during periods of high census, staffing shortages, or unplanned absences.
• Participate in required weekend, holiday, and on-call rotations in accordance with organizational policy.
• Provide real-time coaching, mentorship, role, and behavioral guidance to assigned staff.
• Engage in regular leader rounding and check-ins with all team members.
• Coach, mentor, and guide performance or behavioral concerns. Escalate significant concerns to the appropriate leader.
• Intervene promptly in staff conflicts, facilitate respectful dialogue, and reinforce behavioral expectations.
• Support onboarding and acclimation of new staff, ensuring role clarity and smooth integration.
• Partner with the Educational Services leaders to identify learning needs and support targeted training or follow-up education.
• Prepare for and conduct annual EMPOWER meetings in accordance with organizational guidelines.
• Lead or support daily huddles and team check-ins to promote operational alignment and readiness.
• Serve as an escalation point for CNA staff regarding workflow questions, care coordination, or task prioritization, and coordinate with operation and education leadership as needed.
• Foster a positive team culture focused on collaboration, accountability, engagement, and recognition.
• Share productivity, performance, and workflow trends with staff to promote awareness and efficiency.
• Implement approved workflow or process changes, providing clarification and reinforcement to ensure consistent practices.
• Identify and communicate workflow barriers, inefficiencies, or safety concerns to leadership.
• Gather staff input and communicate feedback to appropriate leaders to support continuous improvement initiatives.
• Partner with Education and Quality teams on process rollouts or training updates.
