About The Position

Horizon Health is a Critical Access, Rural Health Facility comprised of 25-inpatient beds located in Paris, IL & a multitude of outpatient clinic settings including Family Practice and Specialty Clinics in Paris and surrounding cities. We have been serving residents of Edgar County since 1968 though community education, emergency services, outpatient care and assisted living memory care. Willowbrook is Horizon Health’s newest 14 bed Memory Care Facility that seeks to provide our residents with an experience that makes them feel like they are living right at home. We have the ability to provide more one - one family-oriented care with our specialty trained and dedicated staff. Position Summary: The Certified Nurses Assistant or Resident Assistant is responsible for performance of basic patient care and assistance assigned by and under the direction of Registered Nurse. Interacts as a team with RNs, and LPNs, to provide services to patients and their families. Has contact with adults with Dementia.

Requirements

  • Successful completion of an approved Nursing Assistant Program.
  • Current certification with a State of Illinois for Nursing Assistant required for work or a Resident Assistant.
  • Knowledgeable on universal precautions.
  • Knowledgeable age-specific requirements for patient populations.

Nice To Haves

  • 3-6 months experience as a Certified Nursing Assistant is preferred.

Responsibilities

  • Provides or assists with personal hygiene for patients. Gives baths, shampoos, oral hygiene, back and skin care, toileting, and shaves male patients.
  • Takes and records vital signs, intake and output, and other data (weight, height, etc.) Reports abnormal data, patients problems or complaints and changes in a patient’s status to the nurse assigned to the patient.
  • Changes bed linens, and maintains patient rooms and unit in orderly fashion.
  • Assists patients with meals. Positions patients and trays, and feeds patients who are unable to feed themselves. Documents patient intake appropriately.
  • Follow up on customer complaints as quickly as possible by identifying steps to correct problems, communicating with coordinator/manager, implementing plans, and providing feedback to customers.
  • Answering call lights and advising nurses of needs beyond scope of position.
  • Explains procedures and treatments thoroughly to patients, families and/or significant others before completion.
  • Maintains knowledge of job related duties; infection control, OSHA blood borne pathogens, TB/respiratory protection plan, policies and procedures, Disaster Manual, Safety Manual.
  • Observes patient’s confidentiality by no discussing patient issues except on a need-to-know basis, and not looking up information in computer or on patient’s records.
