Provides resident centered personal care including bathing, toileting, grooming, dressing/undressing, obtaining vital signs, psychosocial support and other personal cares. Assists residents in transferring, repositioning and walking using correct and appropriate transfer techniques and equipment; provides range of motion and passive exercise. Completes tasks related to dining and food preparation. Assists residents with meals and snacks, provides water and supplements as appropriate. Provides a positive dining experience. Provides the right medication to the right person at the right time in the right dose and by the right route ensuring all changes in medication are responded to in a timely manner. Accurately documents outcomes related to resident cares, medications/treatments, behaviors, activities and dietary. Provides housekeeping and laundry services to assist residents with keeping living areas clean and operational; follows consistent hand washing and Personal Protective Equipment use. Provides assistance/documentation with wellness and recreational programs and encourages residents to participate. Must possess a desire to work with residents in sub-acute care and long term care.

Requirements

  • High school graduate or general equivalency diploma (GED) preferred, but not required.
  • Minimum age of 18.
  • Prior clinical or nursing assistant experience in long-term care preferred.
  • Required Certified Nursing Assistant (CNA) certification with the State Board of Nursing, or state certified CNA within four months of date into the position by being enrolled in a training program or in the process of obtaining certification through Endorsement.
  • CNA training and certification required and provided by facility.
  • Satisfactorily completes yearly department specific competency testing and maintains a record of continuing education credits for each year. A minimum of 12 hours continuing education are required annually.
  • Maintains all department specific required certifications.
  • When applicable, for applicable states, certified and registered with the Unlicensed Assistive Person (UAP) Registry.
  • Dependent on facility and required responsibilities, this role may classify as a Sanford Category II Driver requiring a motor vehicle report and proof of valid driver’s license.
  • Also must be medically qualified to perform the essential driving functions of this position as per our Sanford Driving Policy per the leaders request.
