About The Position

Performs age and developmentally appropriate nursing related tasks and services delegated by a licensed nurse to adult and geriatric patient/resident populations. After appropriate delegation by the supervising nurse, the nursing assistant shall utilize knowledge of resident rights, legal/ethical concepts, communication skills, safety and infection control while performing the following: basic nursing skills, personal care skills, basic restorative skills, mental health and psychosocial skills, communication skills and nursing team member skills. Delivers resident patient care according to the Scope of Service for the Living Center. Fosters a culture of patient/resident safety on the unit. Practices within the scope of an CNA and compiles with standards of practice for long term care facilities.

Requirements

  • Must have completed an accredited CNA course and have current WY CNA license.
  • Effective and professional communication skills.
  • Good customer service skills.
  • WY CNA required at time of hire and upon expiration of the credentials.
  • BLS required at time of hire.

Responsibilities

  • Provides age and developmentally appropriate personal care to residents (eg. peri care, oral care, linen changes, dressing), encouraging resident choices and independence within resident’s abilities.
  • Provides interventions to reduce loneliness, helplessness and boredom and to improve resident’s quality of life and self worth. Listens and communicates respectfully with residents and families. Encourages residents to communicate opinions, needs, and choices. Consistently treats residents with respect and provides interventions to maintain resident dignity.
  • Follows plan of care as noted on daily mini-care-plan sheet and gives input to nurses on any changes that should be made to further individualize plan of care. Prepares residents for meals, offering choices, distributing food and assisting residents as needed. Provides a meaningful dining experience for residents by engaging them in conversation and creating a positive dining environment. Knows resident diet and offers snacks and appropriate hydration to residents. Consistently documents meal and fluid intake.
  • Makes rounds to anticipate resident needs. Promptly responds to call lights and all other alarms. Assures for resident safety including: fall alarm in place and working; call light within reach; staying with resident as required; using gait belt.
  • Completes resident positioning, lifting, turning, and ambulating according to standards of care and resident’s plan of care using proper equipment and tools to facilitate safe care for both the resident and employee. Collaborates in the implementation of rehab goals such as ROJM, ambulation, positioning, and turning.
  • Consistently follows appropriate infection control practices including hand washing, linen handling, glove use, catheter care, and isolation procedures.
  • Assists in maintaining a safe and clean environment for residents by reporting malfunctioning equipment, environmental hazards, cleans equipment, keeps hallways clear, and equipment in good working order. Implements fire and disaster plan correctly.
  • Keeps residents’ rooms neat, safe, and clean. Handles resident’s personal belongings properly including prosthetic devices, hearing aides, dentures, glasses, and clothes.
  • Checks oxygen for correct flow and makes certain that oxygen is available to meet resident needs.
  • Communicates to nursing staff any changes in resident condition including but not limited to mental status, mobility, positioning needs, skin condition, dietary intake, participation in care.
  • Consistently communicates effectively with team members to effectively address resident needs and implement the plan of care. This includes communication with members of the interdisciplinary team on all shifts.
  • Consistently and accurately completes required documentation for dietary intake, I and O, and aide duties.
  • Actively participates in culture change and implementation of the Eden philosophy of care.
  • Prepares residents for participation in activities and participates with the resident in the LC activity program.
  • Performs fundamentals of customer service such as answering phones, paging, and relaying messages.
  • Assists in performance improvement activities in order to improve resident care and professional practice including completing audits and problem solving to improve clinical processes.
  • Keeps certifications, reorientation and continuing education current. Maintains, develops and updates skills to implement quality care and service to residents and families.
