About The Position

Under the general direction and supervision of long term care R.N. or L.P.N. the long term care C.N.A. provides personal care and assistance to residents to re-establish activities of daily living. This care is provided in accordance with a plan of care that is prescribed by the physician and written by an interdisciplinary team. Requirements

  • Must be certified, or registered by the Illinois Department of Registration as a Nursing Assistant or have documentation of successful completion of a training course approved by another state as evidenced by a diploma or certification; be in a RN/LPN approved course, or have the potential to complete the approved orientation course and enter a CNA training course within 180 days of hire.
  • Interested and empathetic attitude toward caring for the sick elderly, and disabled.

Responsibilities

  • Collects data regarding resident care needs and response to nursing care provided.
  • Reviews the care plan with RN/LPN and identifies potential changes.
  • Evaluates the effectiveness of care provided and reports to the primary RN/LPN.
  • Demonstrates competency in selected psychomotor skills.
  • Implements appropriate measures to meet the resident/SO learning needs.
  • Performs documentation duties in the resident record which is timely, accurate, and concise.
  • Demonstrates awareness and sensitivity to residents/SO rights, as identified by the Patient Bill of Rights.
  • Functions with an awareness and application of safety and infection control issues as identified in the facility.
  • Performs efficiently in emergency resident situations following established protocols, remaining calm, informing appropriate persons, and documenting events.
  • Demonstrates sound knowledge base and actions in the care and decision making for designated resident population.
  • Demonstrates an awareness of self-responsibility and accountability for own skills and actions within scope of facility’s policies and procedures.
  • Participates actively in staff development and orientation activities for the unit.
  • Demonstrates self-directed learning and participation of continuing education to meet own development needs.
  • Seeks validation of knowledge base and skill level and assertively seeks guidance in areas of question.
  • Demonstrates awareness of legal issues in all aspects of resident care and unit functions and strives to manage situations in a reduced risk manner.
  • Participates in development and attainment of unit and division of nursing goals.
  • Participates in unit and division of nursing PI activities.
  • Demonstrates effective communication methods and skills, using lines of authority appropriately.
  • Uses effective working relationships with all health care team members, residents/so and staff.
  • Remains flexible in staffing patterns and resolution of staffing conflicts: participates in temporary assignment measures of division of nursing.
