About The Position

Corewell Health Hospice specializes in providing end of life care and symptom management to individuals with a life expectancy of six month or less, serving individuals in their home, nursing facilities, and in the hospital. In collaboration with the entire interdisciplinary team, assess, plan, implement, coordinate, monitor and evaluate the strategies and services required to meet a patient/family's end-of-life care needs. An outcome-based plan of care is created and continually adjusted through utilization of the nursing process, relationship centered care principles and holistic philosophy assuring the changing needs of the patient/family are met while facilitating a symptom-free, peaceful, sacred death. Acts as a team member in the delivery of quality, direct patient care. Performs basic skills per department guidelines and communicates observations to members of the health care team. Provides direct patient care under the direction of the Unit Manager or Licensed Nurse, consistent with policies and procedures of the unit. Responsible for direct day-to-day care of assigned residents/patients including dressing, bathing, transporting and feeding. Documents care provided as directed in a manner consistent with guidelines. Requirements

• High School Diploma or equivalent
• State of Michigan approved CNA Training Course
• Driver's License within the State of Michigan
• CRT - Nursing Assistant – Michigan Department of Licensing & Regulatory Affairs, Bureau of Community and Health Systems upon hire
