CNA - Home Hospice, Full Time

About The Position

Rainbow Community Care is currently accepting applications for dedicated home hospice CNAs. Use your skills to provide high quality home care to terminally ill patients. The position involves working in different working environments, including patients’ homes and healthcare facilities.

Requirements

Current Wisconsin Nursing Assistant Certification is required.
Valid Wisconsin driver’s license and proof of current auto insurance are required upon hire and must be maintained.
Must be able to drive to and from other office/meeting locations as necessary.
Must be able to maintain healthy boundaries and positive interpersonal relationships.

Nice To Haves

Minimum of 6 months CNA experience preferred.
Hospice experience preferred.

Responsibilities

Assists with ambulation and transfers of patient.
Maintains a clean, safe and comfortable environment.
Prepares meals and assists patients with eating.
Gives emotional and psychological support to the patient/family.
Reports status/observations of assigned patients according to RCC guidelines.
Promotes patients’ independence.
Maintains up-to-date patient records and documents care given and patient condition for each visit.
Assists the patient in activities of daily living including personal grooming, feeding, drinking, toileting, bathing, skin care, nail care, dressing, transferring and walking.
Light housekeeping duties as assigned.
Other duties and projects as assigned.
