About The Position

The Certified Nursing Assistant (CNA), under the supervision of a Registered Nurse or Therapist, performs various personal care services for patients in the home setting using a patient specific care plan to facilitate the patient's self-care and independence with activities of daily living. Responsible for observing patients, reporting these observations, and documenting observations and care performed. Performs other duties as assigned.

Requirements

• High School Diploma or GED Equivalent.
• One (1) year of personal care experience.
• State certification for Home Health Aide training program and competency and is listed on the South Carolina state registry.
• Valid SC Driver's License with good driving record.
• CPR certification.

Responsibilities

• Adds to the care plan established by the RN or Therapist; assists patient/family as needed.
• Notifies RN or Therapist when updates to care plan are needed.
• Assists patients with personal care activities including but not limited to: bathing, dressing, grooming, skin care, hair care, nail care, feeding, oral hygiene, and shaving.
• Toilets patients and assists with bedpans and urinals; provides catheter care, empty catheter bag, if applicable.
• Assists patients with use of cane or walker, special eating utensils, special techniques, and equipment for personal hygiene.
• Documents care performed as outlined on care plan in the electronic medical record.
• Reports all changes in patient's condition (physical and/or mental) and environmental changes to case manager.
• Assists family members with and provides training on lifting, turning, re-positioning, and transferring patient.
• Provides emotional support to patients and/or family members.
• Maintains a safe, secure, and healthy patient environment by following asepsis standards and policies.
• Participates in 12 hours of in-service training.
• Performs other job duties as assigned.
