About The Position

A Home Health Aide (HHA) provides direct patient care to patients under the direction of the RN and according to the Aide Plan of Care (POC). Responsibilities include assisting patients with self-administered medications, taking vital signs, helping maintain personal hygiene, assisting with feeding, toileting, mobility, and documenting care provided. The HHA also practices acceptable infection control principles and assists with household duties as needed.

Requirements

  • High school diploma or equivalent.
  • Completion of Certified Nursing Assistant or Certified Home Health Aide Program within the last 24 months.
  • Must meet applicable state certification requirements.
  • A valid driver's license, auto insurance, and reliable transportation are required.
  • Must be in good standing on the HHA Registry (if applicable) and have completed HHA/CNA course to work for a Medicare certified agency.
  • At least one year experience in the last 24 months as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, or home health/hospice agency.

Responsibilities

  • Provides direct patient care to patients under the direction of the RN and according to the Aide Plan of Care (POC).
  • Assists the patient with self-administered medications by opening bottle caps, reading labels, checking doses, and observing medication intake.
  • Takes accurate temperature, pulse, and blood pressure measurements and reports abnormal results to the supervisor.
  • Helps patients maintain good personal hygiene by performing or supervising bathing, grooming, skin care, shaving, oral care, and nail/foot care.
  • Assists in feeding patients and communicates basic principles of nutrition.
  • Assists with patient toileting, including the use of bedpans and changing catheter bags.
  • Provides skills to assist with patient mobility, exercises, positioning, transfers, and ambulation.
  • Documents interactions with patients, caregivers, doctors, and staff members appropriately and thoroughly.
  • Practices acceptable infection control principles and provides a clean, safe, and comfortable environment.
  • Assists with household duties including light laundry, meal preparation, and housekeeping.
