About The Position

The Certified Nurse Assistant (CNA) position at Trinity Health At Home involves providing essential paraprofessional services to patients in their homes. The role requires adherence to physician orders and compliance with state regulations, ensuring high-quality care and support for clients in a home health setting.

Requirements

• High School diploma or GED required.
• Active CT, CNA certification in good standing.
• Completion of federally compliant training program and successful completion of a competency evaluation program.
• Documentation of one year's employment as an aide in an institution, home care, or hospice setting.
• Must have current Driver's license and reliable transportation to and from work site.

Responsibilities

• Follows care plan developed by RN.
• Reports client changes in condition to RN, as described on care plan.
• Assists clients with personal care activities including bathing, mouth care, denture care, skin care, grooming, bathroom privileges, transferring, ambulation, and dressing and undressing activities.
• Assists clients with household tasks including planning and preparing meals, feeding, light housekeeping, changing linens, making bed, and laundering clothes and bedding.
• Measures and records client's oral, rectal and axillary temperatures, pulse respiration and blood pressure as ordered.
• Performs range of motion and other simple procedures as an extension of therapy services as assigned.
• Assists clients in self-administration of medication as assigned by an RN.
