About The Position

The ProHealth family includes various facilities such as Waukesha Memorial Hospital, Oconomowoc Memorial Hospital, the Rehabilitation Hospital of Wisconsin, ProHealth Medical Group clinics, AngelsGrace Hospice, ProHealth Home Care & Hospice, West Wood Health & Fitness Center, and Regency Senior Communities. In this entry-level position, you will work 12-hour shifts, three nights a week from 7 PM to 7 AM, rotating weekdays and every third weekend with two other peers, including holiday rotations. The work location will be at Angels Grace Hospice. A $2,000 sign-on bonus is available for Certified Nursing Assistants (C.N.A.s) tied to a one-year commitment. Your primary responsibilities will include answering all ProHealth Home Care & Home Hospice phone calls and triaging them accurately under the supervision of a registered nurse. You will also provide community resource information to callers, coordinate projects related to regulatory annual requirements, audits, and patient processes, and complete intake functions while providing CNA support to the staff and patients at AngelsGrace Hospice.
