About The Position
AW Health Care is seeking a dedicated and proactive CNA Home Care Field Care Coordinator (FCC) to join our team. In this role, you will work closely with the Case Manager to ensure the well-being of our clients, both Medicaid and Private Duty, while supporting caregivers and families. You will assist in a variety of tasks, including assessments, care plan creation, training, and documentation. The ideal candidate will have excellent communication skills, a passion for helping others, and the ability to manage multiple tasks effectively. The territory for this FCC is North, Central and South St Louis City and County.
